For ease and convenience, Care Eleven Health offers online bill pay through our payment portal. We accept Visa, MasterCard, Discover, and American Express, and payments are normally processed within 24 hours. Care Eleven Health participates with all major health insurance plans. If you are uncertain as to whether your plan covers services from Care Eleven Health, please call your insurance company first in order to verify your coverage. Currently, we accept the following insurance plans:

In addition, please note that insurance plans are subject to change without notice. Many plans require co-payments, deductibles, or co-insurance payments. All payments are due at the time of service. Also, your insurance may confirm benefits however, confirmation of benefits does not guarantee payment, and you are fiscally responsible for any unpaid balance for all services rendered.
No insurance? No problem!
Care Eleven Health offers a self-pay standard office visit for $100.
The self-pay fee covers a provider visit which includes an evaluation of your medical complaints and the corresponding treatments. If your visit requires additional services such as x-rays, labs, and vaccinations then additional fees will apply.
