Rooted in our community, Care Eleven Health built an integrated healthcare model that provides holistic care to our patients through every life cycle and supports every possible need, including primary care, urgent care, behavioral health, and school health with support for social determinants of health. Our integrated health network creates meaningful change for our patients and the communities we serve.

Care Eleven started in New Orleans’ Bywater neighborhood, offering treatment for everyday urgencies. Over time, our services expanded to include school health clinics and now extend across multiple parishes including Orleans, St. Tammany, St. Bernard, and others.
